• Donor # (for your account)
• Missionary Name (First Initial. Last Name)
• Designated Dollar Amount
  • If split evenly among multiple missionaries, you may write the amount one time designating it “for each.”
  • If your designation is too long to clearly read in the memo line, please include it on a separate piece of paper.
